Supporting our community through local area co-ordination.
The aim of Connected Communities is to bring a community-focused approach to supporting residents across Elmbridge. At the heart of this model will be a dedicated team of Local Area Co-ordinators (LACs), employed by the council to work directly with individuals and communities.
LACs will provide one-to-one support for people with complex needs, helping them build independence and connect with local services, groups, and charities. Each LAC will work closely with individuals to co-create action plans tailored to their goals and circumstances. They will ‘walk’ alongside them on their journey to greater wellbeing and community connection.
Each LAC officer will be place-based, assigned to a specific area within the borough. This local focus means they’ll have deep knowledge of their area’s needs, strengths and available support networks.
They’ll also build strong relationships with key community stakeholders such schools, GPs, charities, and local organisations to ensure a joined-up approach to support.
LACs will share insights and local emerging needs with the council, helping to shape services and fill gaps in provision.
By being out in the community, LACs will give the council a clearer picture of what’s happening on the ground. This will enable the council to act as a facilitator of support, whether that’s adapting services or enabling new initiatives in response to local concerns.
